- CSSPS Placement Form – Official placement form for SHS/SHTS.
- BECE Result Slip – The original result slip provided by WAEC.
- Enrollment Form – This form includes the essential Enrolment Code.
- Birth Certificate – Valid proof of age and identification.
- Two Passport-Sized Photos – Recent, clear photos for school records.
The Enrolment Code found on the Enrollment Form is mandatory for admission.

- Visit Your School’s Admission Page: Access the school’s official website and click the Admission tab. Scroll down to find the input field.
- Enter Your Index Number: After entering your Index Number, click Proceed to view your admission information and initiate payment.
- Make Payment (GHC1): Schools require an initial payment of GHC1, which is processed through MoMo. Follow these steps:
- Dial *928*990# on your mobile device.
- Enter your school’s unique abbreviation (e.g., “apgss”).
- Type in your Index Number and wait for the payment prompt or approve the transaction through your MoMo approvals.
- Confirm with your MoMo PIN to complete the transaction.
- Continue Admission: Upon successful payment, return to the admission page and click Continue. A verification code will be sent to your phone.
- Verify Your Code: Enter the code and click Verify. This finalizes the verification process.
- Complete Admission and Download Documents: You will now be redirected to the school’s admission dashboard. Complete any remaining fields and download all necessary documents, including the Prospectus, School Rules, and other vital information.

DO THE FOLLOWING WHEN YOU ENTER YOUR INDEX NUMBER AND SYSTEM SAYS INDEX NUMBER CANNOT BE FOUND
1.First click on the GET CODE button next to PROCEED.
2.Get your CODE by entering your FULL NAME.
3. When the search is completed, you will see your CODE at the first column in the table.
4.You then enter your FULL INDEX NUMBER and your CODE in the spaces that will be displayed.
5.Click on the Save Changes then enter your FULL INDEX NUMBER and click on PROCEED to continue the process.
