Guidelines on How to Fill and Submit the GESOPS Contributor Enrollment Form

The article relates to guidelines for the filling and submission of the Ghana Education Service Occupational Pension Scheme (GESOPS) contributor enrollment form.

Procedure:
1. Fill in the necessary personal details and fix your passport picture on the right top corner of the front page.

  1. Turn to the back page for the declaration.
  2. Part II: Employment Details (To be completed by Head of school). That of the head of the school should be completed by his or her SISO.

  1. Declaration by GES Supervisor, should also be completed by the Head of school.

  2. Attach a photocopy of your National ID card to the form, for onward submission to the GLICo office by the Head of your school.

NOTICE:
You are kindly advised to use your National ID for the identification details.
